COVID Vaccine Pre-Registration for Phase 1 Priority Groups

Dear Patients,

Good news! COVID vaccines are finally starting to come in...

Eligible priority groups under Phase 1 of the provincial vaccination plan are now welcome to pre-register for the vaccine. This includes all patients 80 years old or older.

**Please note: Pre-registration does not guarantee a vaccine appointment, nor will you be contacted immediately to book an appointment. Vaccine appointments will be booked based on vaccine availability and eligibility.

When you pre-register, you will be asked to select either email, text or telephone for your preferred method of communication. When a vaccine is available, you will be contacted through the method selected. Eligibility will be confirmed before any appointment is booked.

Below are the pre-registration websites for North York General Hospital and Sunnybrook Health Sciences Centre, where you can find more information about adding your name to their respective vaccine clinic pre-registration lists:

North York General Hospital

Pre-registration Criteria:

1.People who are aged 80 years and older who are:

  • Living independently in the community
  • Attend hospitals for treatments such as dialysis or chemotherapy
  • Living in a congregate care setting such as assisting living

2. Health care workers who are identified as Highest Priority and Very High Priority risk categories according to Ontario Ministry of Health Prioritization Guidelines for Health Care Workers. Pages 8-10 of the Prioritization Guidelines for Health Care Workers lists the frontline workers that fall into the Highest Priority and Very High Priority categories.

Sunnybrook Health Sciences Centre

Pre-registration Criteria: 

  • 1. Toronto residents 70 years of age and older (born in 1951 or earlier)
  • 2. Indigenous adults
  • 3. Adults who receive home care
  • 4.Health-care workers (a person who works in a health-care setting in any capacity)

(Please note: This is obviously not an exhaustive list of all vaccine clinic locations. Rather, we have simply listed the two hospitals with which our clinic is most closely connected, by geography as well professional affiliation. Depending on where you live, other hospitals and/or Public Health Units may be more convenient or appropriate for you. Should that be the case, we would encourage you to look into other local options.)

We strongly recommend pre-registration for people in all eligible priority groups, as our community strives to protect those most at risk of COVID-19. As more vaccines become available, more groups will be able to register following the provincial vaccination rollout plan.
