Dear Patients,

It's moving time!

We ask that all patients take note of the following upcoming important office dates in relation to our relocation, and appreciate your patience as we navigate the move to our new home.

IMPORTANT UPCOMING OFFICE DATES

• September 19 - Sep 21 (Thu-Sat) - CLOSED FOR RELOCATION
• September 23 (Monday) - Soft Reopening - office phones/email open
• September 24 (Tuesday) - Full Reopening

Please note that during our relocation (Sep 19-23) our physicians will remain accessible normally via DSG Secure Email, however access to patient charts may at times be limited.

NEW OFFICE LOCATION (effective Sep 24):

The building entrance is beside Tim Horton's, on the west side of Yonge Street just north of Lawrence Avenue (and the Yonge/Lawrence subway station). Elevators are just inside the entrance; our suite is on the 6th floor at the end of the hallway.
